Eligibility
nOur services are available to individuals who are at least 18 years of age. Minors (under 18) may receive certain services, such as teen acne treatment, only when accompanied by a parent or legal guardian who provides written consent and is present during the consultation and treatment.n
Services
nReviv Beauty Med Spa provides aesthetic and medical spa services including, but not limited to, neurotoxin injections (Botox, Jeuveau, Xeomin, Daxxify), dermal fillers, microneedling, chemical peels, medical weight loss, IV therapy, and related wellness services.n
nAll medical aesthetic services are provided under the supervision of Kristen-Anne Thomas, FNP-BC, a board-certified Family Nurse Practitioner.n

Appointments and Cancellations

Cancellation Policy
nWe require at least 24 hours’ notice for appointment cancellations or rescheduling. n
- Cancellations made with less than 24 hours’ notice may be subject to a $50 cancellation fee.
- No-shows (failure to arrive for a scheduled appointment without notice) will be charged a $50 no-show fee.
nWe understand that emergencies happen. Please contact us as soon as possible if you need to cancel or reschedule.n
Late Arrivals
nIf you arrive late for your appointment, we will do our best to accommodate you. However, your treatment time may be shortened, or we may need to reschedule your appointment to ensure all patients receive their full treatment time.n
Pricing and Payment
Pricing
nPrices for services are listed on our website and are subject to change without notice. The final price for your treatment will be confirmed during your consultation.n
Payment
nPayment is due at the time of service. We accept cash, credit cards, and financing through CareCredit, Allu0113 Payment Plans, Allu0113, and Alphaeon Credit.n
Packages
nService packages are:n
- Non-refundable once purchased
- Non-transferable to other individuals
- Valid for 12 months from the date of purchase u2014 unused portions expire after 12 months
Prescriptions
nPrescription medications (including but not limited to weight loss medications, tretinoin, and compounded prescriptions) are non-refundable once dispensed. Due to safety and regulatory requirements, we cannot accept returns of prescription medications.n
Refund Policy
All sales are final. We do not offer refunds for services rendered or products purchased.nn
nIf you are dissatisfied with a treatment, please contact us within 14 days of your appointment. We will work with you to address your concerns, which may include a complimentary follow-up appointment or corrective treatment at our discretion.n
Consent to Treatment
nBefore receiving any treatment, you will be required to complete intake forms and sign informed consent documents. These documents will explain the nature of the treatment, expected results, potential risks and side effects, and alternative options. You have the right to ask questions and receive answers before consenting to any treatment.n
Health and Safety
nFor your safety, you must provide accurate and complete health information, including your medical history, current medications, allergies, and any relevant health conditions. Failure to disclose relevant health information may result in complications and will release Reviv Beauty from liability.n
nCertain treatments may not be appropriate for individuals with specific health conditions, pregnant or nursing women, or those taking certain medications. Final determination of treatment eligibility will be made by our medical provider.n

nAll content on the revivbeauty.com website, including text, images, logos, graphics, and software, is the property of Reviv Beauty LLC or its content suppliers and is protected by United States and international copyright, trademark, and other intellectual property laws.n

Dispute Resolution and Arbitration
Informal Resolution
nIf you have a dispute with Reviv Beauty, please contact us first at info@revivbeauty.com. We will attempt to resolve the dispute informally within 30 days.n
Binding Arbitration
nIf we cannot resolve the dispute informally, you and Reviv Beauty agree to resolve any claims through binding arbitration rather than in court, except that either party may bring a claim in small claims court if it qualifies.n
nArbitration will be conducted by the American Arbitration Association (AAA) under its Consumer Arbitration Rules. Arbitration will take place in Seminole County, Florida. The arbitrator’s decision will be final and binding.n
Class Action Waiver
nYou and Reviv Beauty agree that any dispute resolution proceedings will be conducted only on an individual basis and not as a class, consolidated, or representative action.n
Small Claims Exception
nEither party may bring an individual action in small claims court for disputes within the court’s jurisdictional limits (generally $5,000 or less in Florida).n
Governing Law
nThese Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any legal action not subject to arbitration shall be brought in the state or federal courts located in Seminole County, Florida.n
